Why do I need to fill out a Consent to Disclose form?

A Consent to Disclose form is standard documentation to uphold your confidentiality and privacy.

Our professional code of ethics, under the Ontario College of Social Workers and Social Service Workers, clearly defines that all information you share with your therapist is confidential. It will also not be communicated to anyone, directly or indirectly, without your informed and written consent.

Verbal consent (by phone) or written communication (by e-mail), cannot be used as consent to disclose. Instead, we require a form to be completed as documentation. The form is called a Consent to Disclose form.

To request that we release any information related to your therapy sessions, you can complete a Consent to Disclose form. On the form, you can place limitations on what you permit to be shared, and who it is shared with. You can also refuse to sign the form entirely, as it is completely voluntary. 


Here are some examples of where a Consent to Disclose Form in required:

  • To confirm or deny that you are receiving services at our clinic
  • To release session information, such as chart notes and billing details, to anyone other than you
  • To have another individual pay for your sessions, and/or receive the session receipts and invoices 
  • To have another individual manage your bookings, cancellations, and rescheduled sessions
  • To verify session details with your Insurance provider
